I think generally people pay more for 5-year managers, salesperson, 
project managers, 
con-sultants  or Analysts,  etc than programmers. 
imho programming is grunt work. Lots of hard work, not so much pay. 
(Pure 
programming only, w/o  s/w analysis or s/w architecturing or design). 
Then, maybe it 
could be the inefficiency of the language, tools etc. ('cos  I mostly code 
with a text editor 
and procedural ).

I have not read the MMM. How much do you cost a man-month of a 
programmer ?  
Compare with a Proj Manager ? Consultant??? I know some 
consultants are charging 
2K a day. and that is considered cheap.
